Inspections of Cross Pointe Care CenterCross Pointe Care Center gets an average rating for health care. This nursing home last received a health inspection on 08/31/2017, during which inspectors cites six citations, which is higher than the 4.6 violations which nursing homes in Florida average.
Inspectors found Cross Pointe Care Center did not adequately “maintain an environment free of accidents hazard, pests, and provide personal space accommodation for 1 of 30 sampled residents.” The inspection report notes the presence of two dead insects and two small “roach-like” live insects, debris on a toilet seat, brown stains near the toilet, uncovered lighting fixtures in the bathroom, broken tiles, and other issues. In addition, dead ants were found in one of the bathtubs and a hole in the wall was found.
The inspection further concluded that the “facility failed to ensure a comprehensive care plan was developed to include appropriate monitoring, safety measures, and re-evaluation for the continued necessity of a 4-point wheelchair vest harness” for one resident. Inspectors also concluded this nursing home did not always “store, prepare, distribute, and serve food under sanitary conditions to help prevent food borne illness.” During at inspection in 2017, steam-pans were not stored so they could dry and dish-machine sanitizer tests were not stored where they should be. Inspectors noted some brooms were not stored off the floor and single-use cups were not covered. Dried food and dust was noted on some kitchen equipment.
Inspectors also reported that Cross Pointe Care Center failed to “have a program that investigates, controls and keeps infection from spreading.” The inspection report found that medication carts were not always covered, the laundry facilities were not kept in good repair and cleanliness and that proper hand washing was not always observed during administration of patient medication. Inspectors also noted a blood pressure cuff was not cleaned off before being used on another patient.
The health inspection report of 08/31/2017 also noted the nursing home should “make sure that a working call system is available in each resident's room or bathroom,” since such communications were not adequate in 13 of the 34 rooms inspected. Inspectors further found that in one of 5 patients reviewed, the nursing home did not keep full clinical records and in seven instances did not document the specific location of a medication. The last fire safety inspection for this nursing home occurred on 08/29/2017. At that time, one citation was noted. This is significantly less than the three average citations issued to nursing homes across the country.
