Palmetto Care Center

Palmetto Care Center is a for-profit Florida nursing home with 90 certified beds. This home participates in Medicare and Medicaid and is not in a hospital and not in a Continuing Care Retirement Community (CCRC). Inspections of Palmetto Care Center

Overall, Palmetto Care Center gets a below average rating from Medicare Nursing Home Compare. Between 2015 and 2018, the facility received one complaint leading to a citation. As of summer 2018, the last health inspection of this nursing home was on 11/28/2017. The inspection report listed 9 citations, higher than the Florida average of 4.5.

Health inspectors inspecting the facility on 11/28/2017 found the nursing home did not provide quarterly statements to one resident, even though the facility should help residents hold, manage and secure the money deposited with the nursing home. Inspectors also found that the facility failed to provide assistance for at least one resident during bathing and failed to implement policies which would help prevent theft as well as abuse and neglect of residents. Inspectors observed residents with discoloration on their skin and through interviews determined some multiple incidents had occurred. These incidents resulted in injury to residents.

The inspection report of 11/28/2017 also concluded that Palmetto Care Center “failed to develop a care plan related to contractures and range of motion (ROM)” and did not provide the care indicated on each patient’s written care plan. Inspectors found residents with limited mobility did not always get the services and treatment needed to reduce further decreases in motion. In addition, residents were not always given the right support, according to inspectors, to prevent accidents and dangers during bathing. The inspection report also revealed reservations about how food was prepared and how ovens were maintained. Inspectors concluded that the double ovens had worn walls with rust-like substances, and the oven knobs and handles had similar discoloration. Inspectors also noted in their report that this nursing home failed to provide the correct level of Hospice Services and support for at least one resident.

A fire safety inspection was conducted at Palmetto Care Center on 10/27/2017 and one citation was noted, which is lower than the two citations which are average for Florida nursing homes.
