Inspections of Signature Healthcare Center of WaterfordThe health inspection rating given to this nursing home by the U.S. Centers for Medicare & Medicaid Services is “below average.” As of September 2018, the last inspection report available for Signature Healthcare Center of Waterford is dated 01/16/2018 and nine citations were listed on that report. During that inspections, inspectors concluded that “the facility failed to maintain resident rooms in safe, clean and comfortable condition.” In four rooms, there were various concerns, including a hole in the wall, stains on a pillow, marks on the plaster and ceiling, and other concerns. Inspectors also found the nursing home did not “implement care plans” for two out of 35 residents observed. Some of those who did not have their care plans followed included residents with skin conditions and pressure ulcers.
In addition, the inspection found that Signature Healthcare Center of Waterford did not “follow professional nursing standards of practice for an unresponsive resident.” The resident had a hypoglycemic episode and inspectors concluded staff did not correctly document blood sugar results or the offering of simple carbohydrates based on the facility Hypoglycemic Management Protocol. In one instance, inspectors found a resident with Diabetes Mellitus did not get appropriate care, as defined by the hypoglycemic diabetic protocol.
Inspectors compiling the 01/16/2018 report also found the nursing home “failed to provide treatment and services” to help a high-risk patient avoid a “facility acquired pressure ulcer.” According to inspectors, a lower-mobility patient was found with a bandage which was not dated to indicate when it was last changed. The patient had a blister formed on the foot and his legs were not elevated when he was in his wheelchair.
The same report indicated that for two of five observed patients “the facility failed to adequately assess and administer in a timely manner pharmacological and non-pharmacological treatment/interventions” for pain management. In both cases, the patients had obvious pain, crying out, grimacing, and crying due to pain. Investigators also concluded the nursing home “failed to ensure effective administration” and did not “ensure staff was following professional nursing standards of practice” while also failing to make sure staff followed the nursing home’s protocol. The inspection also found that the nursing home showed a “failure to ensure emergency medication was readily available to staff.” Quality assurance and implementation of effective plans of action to address concerns was also cited by inspectors as areas of note. Some bandages at the facility were also not disposed of as required by biohazard rules.
